1 – Independent Contractor Agreement

2 – Current W-9

3 – Copy of Notary Commission (stamp not acceptable)

4 – Copy of Driver’s License (must be color, front and back)

5 – Copy of Notary Bond for $10k (per Pennsylvania state minimum)

6 – Copy of E&O Policy with $25k minimum

7 – Copy of Background Check
